- Development of Investigation Teams capability to deliver quality investigations for all levels of incidents that incident owners can call on
- Providing appropriate training courses for above groups
- Researching the available market tools and proposing optimum option for effective identification and analysis of incident causation to suit client needs
- Developing the capability for effective incident investigation review
- Developing the capability for effective Safety Review for non routine activities, or start up after operational incidents
Improving Incident Classification and Reporting
- Delivering fit for purpose improvement to the clients incident reporting system for integration across all their business units
- Reviewing and updating incident reporting and classification procedures
- Rolling out awareness workshops across the company’s UK and European regions
Causal Analysis
- Reviewing clients incident reporting system and assessing quality of incidents actual and potential classification
- Reviewing the quality of identified immediate and latent failures
- Reclassifying above using Basic Risk Factors for giving consistency and aiding causal analysis
- Reviewing quality of assigned actions for preventing reoccurrence
- Presenting findings and recommendations to senior management
